We are delighted that you are interested in joining us for High Holy Days! All are welcome to attend
some or all our services. Visitors, including visitors of Tree of Life members, need to register for
services.  Members of Tree of Life are automatically registered through their renewal and do not
need to register for services with this form.

We ask that you check in prior to services at the Welcome table in the lobby.  We require photo ID
for all adult visitors.

We offer complimentary childcare during Rosh Hashanah Day Morning Service on
Thursday, October 3 and Yom Kippur Morning Service on Saturday, October 12.  Children in
preschool through the fifth grade can enjoy crafts and playtime.  Registration is required.

SUPPORTING TREE OF LIFE

We appreciate your donation to Tree of Life.  Because we do not charge for High Holy Days tickets,
your generosity will help us continue to provide a community for many adults and youth who rely
on us for engagement, support, and nurturing of their Jewish identity.

A suggested donation for High Holy Days is $36/family or $18/individual per
service, but we appreciate whatever amount you can contribute.  College students and military
members are TOL Guests. This form will allow you to pay with a credit/debit card. After payment,
